Please click on each of the items below in order to download the forms, fill them out with a pen and return to our office at your earliest convenience. Your child will be considered registered once the registration fee is paid and all the paperwork is complete and returned to us. Your child will not be able to start on the agreed upon date until this paperwork is completely filled out. Thank you for your cooperation.
